To contribute to the growth and success of the Company by assisting in the development of, and working as a member of a multi-disciplinary team to deliver effective and high quality care to patients, by facilitating discharge from hospital and providing care for patients at home
NB: This post will require a flexible approach to working hours, much like a ward based post with scheduled evening and weekend working on a rotational basis and occasional on call
A dual responsibility role, with internal rotation, to act as case manager workin! g alongside existing discharge coordination teams within the a! cute trusts to facilitate the discharge of excess bed day patients from the acute trusts and the delivery field based nursing care to patients in their own homes
Responsibilities Include:
Participate in setting and maintaining standards of professional competence and current nursing practices. This will require an understanding of and adherence to the NMC Code of Practice
Assist in the development and delivery of a service to identify and discharge excess bed day patients from acute trust, assessing patients and agreeing suitability for either signposting or case management, or for discharge to a home care pathway
Assisting the referrer and associated professionals in planning a smooth transition of patient care into further intermediate or community based services or from the hospital to home
To participation in multidisciplinary team meetings at ward level to identify suitable patients for case management or transfer to the h! ome care pathway
Liaison with ward staff regarding clinical progress of patients in order to indentify suitable patients for case management or transfer to the home care pathway
Delivering patient care in their own homes according to an agreed model of care and care plan
The safe administration of therapies such as antibiotics, hydration, pain control, clinical trials, wound management and assessment
Assisting patients in their activities of daily living
Ensuring all relevant information regarding patients or clients is recorded, reported and where necessary, acted upon to communicate changes to the GP and other team members as appropriate
Ensuring all aspects of care for assigned patients are met, through the monitoring of patient progress according to the care plan
Participating in quality assurance initiatives, including team clinical supervision
Co-ordinating and managing own daily workload
! Contributing to pre-planned pathways and protocols to meet the patient! s needs
Educational
Assisting with the planning of programmes of education â" both induction programmes and local or national study days
Attending and contributing to appropriate conferences, tutorials, study and induction days
Maintaining own professional profile in relation to PREP and attending post-basic courses or study days as agrees with Healthcare at Home to fulfil NMC requirements
General
Having responsibility for the health, safety and welfare of self and others and complying at all times with the requirements of Health and Safety Regulations
To cooperate fully in discharging the organisations polices and procedures with regard to health and safety matters
To immediately report to line manager or other responsible person any shortcomings in health and safety procedures and practice
To report any accidents or incidents to their immediate line manager or safety representative as early ! as possible and submit a completed accident form
To use protective clothing and equipment where provided
Working as part of the Healthcare at Home team and promoting Healthcare at Home at all times
Undertaking such duties as may be required from time to time as are consistent with the responsibilities of the grade
Being flexible with working hours including scheduled weekend work
Experience:
RN part 1 NMC Register
Experience in an acute or community setting (relevant ENB post registration)
Evidence of continuing professional development
Cannulation and phlebotomy skills desirable
Experience in wound care, orthopaedics, respiratory therapies desirable
Personal Skills & Qualities:
To be responsible, without supervision, for the assessment of care needs and the development, implementation and evaluation of programmes of care with an understanding of the NMC Code of P! ractice
The ability to co-ordinate and communicate all aspects! of care and patient plans with the multi-disciplinary ward staff, including health and social care, and the Healthcare at Home multi-disciplinary field based team
To assist in the planning and delivery of training programmes and maintaining supplies in good working order
To work as a flexible team member, showing interest and participation in understanding the business and industry at all times and to be responsible and compliant for Health and Safety Regulations for self and others
To be proactive and motivated responding positively to new challenges
A current and preferably clean driving licence
